The forms listed below form part of the global forms that are available to clients. The forms are subject to client preference and the required forms are added when the implementation team has been advised accordingly.

This information is stored in the payroll system. Employees are required to complete this information accurately.
The employee is required to complete all fields marked as required (red line):
Form I-9
Employers must complete Form I-9 to document verification and employment authorization of each new employee (both citizen and noncitizen) hired after November 6, 1986, to work in the United States. In the Commonwealth of the Northern Mariana Islands (CNMI), employers must complete the Form I-9 to document verification of the identity and employment authorization of each new employee (both citizen and noncitizen) hired after November 27, 2011. Employers should have used Form i-9 CNMI between November 28, 2009 and November 27, 2011.

Authority for Release of Information
With the requirement of maintaining the safety and security of customers, employees and property within companies this form orders a "consumer report" (a background report) to be done on the employee.
The background report may contain information regarding character, general reputation, personal characteristics, mode of living, criminal records, police reports, education requirements, etc. and verification of Social Security Number. The information is available from public sources and private records, investigative consumer reports as well as personal interviews with colleagues, friends, and neighbors.
Employment Agreement
This is a formal agreement that specifies the conditions of the relationship between an employee and an employer including compensation and expectations. Also referred to as employment contracts, they are often executed for a specified period of time, such as one year.
Employee Direct Deposit
This allows the employer to utilize the convenience of direct deposit as a pay method for distribution of pay checks. Filling out this form will allow for the employee’s pay check to be sent via electronic ACH methods to the bank account of your choice.
Allowance is made for up to eight (8) different accounts.
Employees are able to split their wages:
- Full Check - This options allows they entire check to be deposited into one account or the balance of the check if one of the other options are selected.
- Percentage - Allows the employee to deposit a percentage of their check into an account.
- Amount - Allows the employee to deposit a fixed amount into an account.
**NOTE**
If Percentage and/or Amount is selected an employee will be required to have the remaining amount deposited as a full check.
Employee Notice
This form is available for the employee to acknowledge that they have received their information pertaining to the Workers’ Compensation Network.
Federal W-4
Complete this form so that the employer can withhold the correct federal income tax from your pay. Consider completing a new Form W-4 each year and when your personal or financial situation changes.
Employees must complete:
- Section 3 - Select a Marital Status.
- Section 5 - This section can be completed directly or calculated using the worksheet A-H.
- Section 6 - This section is for an additional withholding amount.
Employee Handbook Acknowledgment
Employees sign this acknowledgment after they have downloaded the Employee Handbook.
Should there be an Employee Handbook that has been uploaded, the employee can download it by selecting the download option.
If there is no handbook available, as per the image the employee will be advised accordingly.
Consent to Drug Testing
The consent to drug testing form is signed by an employee who agrees to subject himself/herself to drug testing. The consent to drug testing form is used to obtain the consent from individuals subjected to screening.
Wage Agreement
The wage agreement is an agreement that allows the employer to make deductions from the employee’s wages dependent on the circumstances provided within the wage agreement.
Non-Compete Agreement
The non-compete agreement is an agreement where one party (usually an employee) agrees not to enter into or start a similar profession or trade in competition against another party (usually the employer).
ACA Disclosure Form
This form is signed once an employee has downloaded all the listed notifications.
Veteran Survey
This form fulfills the regulatory intent to gather more information on the number of disabled and veteran workers at the applicant stages.
